FIRST: That at alltime hereinafter mentioned, the plaintiff was domestic corporations existing
under New York State laws, with offices to transact business at:5898 Rome-Taberg Road, Rome,
NY 13440.
SECOND: That at all times hereinafter mentioned, the defendant(s) resided at 61 Bridge St,
Cleveland NY 13042.
THIRD: That on or about and prior to October 8, 2020 the plaintiff at the special instance and
request of defendant and/ or defendant's dependents performed certain work, labor and services
for, and/or sold and delivered certain goods, wares, merchandise, to the defendant and/or
defendant's dependents of the reasonable value and agreed price of $2,444.94.
FOURTH: That the amount presently owed by the defendant to the plaintiff is $2,444.94.
WHEREFORE, plaintiff demands judgment against the defendant for the sum of $2,444.94 with
a per diem interest from October 8, 2020, besides costs and disbursements of this action.
