Plaintiff(s) by the undersigned Attorney, alleges:
1. The Plaintiff is a Federally-Chartered Credit Union, licensed to conduct beiness in New York, with a
principal place of business at 4 Winners Circle , Albany, County of Albany, State of NY.
2. Upon information and belief at all times hereinafter mentioned, the Defendant(s) is/are an individual(s),
with a residence at 16 Pauline Court Rensselaer, NY 12144.
3. That there is due and owing to the Plaintiff by the Defendant(s), the sum of $15,433.13 which includes
principal of $14,607.31, plus accrued but unpaid interest as of July 17, 2020 in the sum of $825.82,
--=alated late charges and miscellaneous fees of $0.00, plus interest at the rate of 9.900% from July 17,
2020 by reason of the Defendant(s) default in the payment pursuant to the terms of a Revolving Credit Card
Account dated November 7, 2012.
4. That although payment has been demanded of the Defeñdant(s) by the Plaintiff, such has been neglected
and refused and/or denied. Plaintiff has been damaged in the total amount of FIFTEEN THOUSAND FOUR
HUNDRED THIRTY THREE AND 13/100 DOLLARS ($15,433.13) plus future interest on the principal
balance from July 17, 2020.
WHEREFORE, Plaintiff demands judgment against the Defendant(s), for the total amount claimed due
herein together with reasonable attorneys fees, ifprovided by contract and allowed by law and the costs and
disburaements of this action, and whatever additional relief the Court deems just, proper and equitable, less
payments made.
