The Plaintiff, NORTHWEST BANK, oy and through their attorneys,
complaining of the aefendant(s) above-named and for their cause of action,
allege as follows:
1. That the Plair-tiff, NORTEWEST BANK, at all times hereinafter
mentioned, is a corporation organized arld existing under the laws of the
Corr.monwealrn of the Sta e of Pennsylvaria, and is dlly aathorized and license
to conduc±_ and transact business withir. tne State of New York, with its
principal place of business located at 100 Liberty Street, Warren,
Pennsylvania 16365.
2. That the Defendant, JAROD T. POCL-N, upon information and
belief, reside at 15 Finch St., Portville, New York 1477C, within the County
of Cat'..araugas.
3. That on aboat Octaber 12, 2019, the Plaintiff ar-d the
Defendant entered into a motor vehicle ir-stallment sale cor-trect

(hereinafter referred to as the "loan agreernent") whereby Plaintiff loaned
to the Defendant, JAROD T. POULIN the sum of S39,601.C0, along with interest
at an annual rate of 7.650 percent, for the purchase of the r_ew 2019 Dodge
Journey (VIN# 3C4PDDGG5KT857496). That rhis loan agreement is attached
hereto and rr.ade a part herecf.
4. That the Plaintiff has performed all of the conditions,
cozenants and promises recuired by them to ne performed in accordance with
the --.erms and cor-ditions under the --oan agreement, with the last scheociec
payment b·y the Defendant to the Plaintiff oeing on cr about February 15,
2020.
5. That the -efendant has brea:ned the loan agreement with the
Plainr-iff by failing and refusing to perform his abligations by not rnaking
payment(s) to tne ?laintiff as provided under the loan agreement.
6. That as of June 2, 2021, a balance of $43,405.17 is owea by the
Defendant to the Plaintiff under the loan agreement, as follows:
Onpaid Principal Ba-ance: $39,415.86
Interest Balance: S 3,796.25
Late Fees due: S 193.06
