AGREEMENT
1. The Parties to this matter are Anthony Jenkins, Jr, (aka “A.J.” ), the father of the child
and Courtney Belloli, the mother of the child.
2. The child who is subject to this Agreement is:
Alyssa Belloli, Date of birth: October 17, 2019.
3. The father resides at 10850 Kyle Road, Bon Aqua, Tennessee.
4. The mother resides at 24 Shawmut Ave, Oak Bluffs, MA.
5. The father shall be adjudicated the father of Alyssa Belloli, Date of birth: October 17,
2019
6. The parties acknowledge that their child is currently in the temporary custody of the
Department of Children and Families (DCF) through a Care and Protection case. The
case was filed solely because of the mother’s actions,
7. The parties agree that the father will have physical custody of the child at the time the
Dukes Juvenile Court transfers custody from DCF. The parties anticipate the Juvenile
Court will transfer custody to the father on July 30, 2021.
8. ‘Because the child will be living with the father in Tennessee, the Juvenile Court needs a
Probate Court custody order to comply with the Interstate Compact on the Placement of
Children (ICPC).
9. The Parties agree that the child will live with the father in Tennessee. They agree that that
father may remove the child from the Commonwealth.
Filed 7/a7/a\ (é)10. The parties shall share legal custody. Any contact between the mother and the child shall
be supervised by the father.
11. The parties agree to add the father to the child’s birth certificate which was issued in
Tennessee.
12. The Father waives child support.
