At alltimes mentioned herein, plaintiff and judgment creditor was and remains a public welfare department located in the County of Lewis and State of New York. 2. On information and belief,defendant and judgment debtor resides in the County of Jefferson and State of New York. 3. On or about June 22, 2011, judgment by default, Index No.: CA2011-000094 was duly entered in favor of this plaintiff and against this defendant for the sum of Seven hundred fifty dollars and 35/100 ($750.35) in the Lewis County Clerk's Office. A copy of said judgment is annexed hereto and marked Exhibit "A". No part thereof has been paid. 4. To date, no payments have been received, leaving the judgment balance remaining due of $750.35 plus interest. 5. Said judgment was duly docketed on June 22, 2011, in the Lewis County Clerk's Office, more than nine years prior to the commencement of this renewal action. 6. This action isbrought under CPLR 5014(1). WHEREFORE plaintiff demands the entry of a renewal of the existing judgment infavor of the plaintiffand against the defendant for the sum remaining due on said judgment with interest,together with costs and disbursements of this action and such other, further and different relief as to thiscourt may seem just.
