complaining (s), respectfully alleges:
1. Plaintiffis a ouipuration authorized to do business in the State of New York. Under present rules plaintiff is
not required to be licensed as a debt Affairs.--
collector by the New York City Department of Consumer
2. That the defendant resides in the in
(s) county which this action is brought.
FOR A FIRST CAUSE OF ACTION
3. Heretofore, the parties entered into an agreement, a copy of which was mailed or delivered tõ
defendant (s) as required law, wherein defendant
by obtained a credit cand, under the terms of which were
they
authorized to, and did, make charge purchases and obtain cash advances and were obligated to reimburse plaintiff
therefor.
4. U pon information and belief, defendant defaulted in
(s) making the payments due therein and owe
presently
a balance of $2,634.44.
FOR A SECOND CAUSE OF ACTION
5. That heretofore, plaintiffrendered to defendant (s) monthly, full,just and true accounts of the indebtedness
due and owing by.defendant as a result of the aforesaid
(s) transaction, which is the sum set forth and said
above,
statements were delivered to defendant without
by (s) objection, resulting in an account stated for the amount
claimed above.
WHEREFORE, plaintiffdemands judgm t against defendant (s), for the sum of $2,634.44 together with costs,
disbursements and statutory post judgment f erest.
