true. My belief as to those matters therein not stated upon knowledge, is based upon facts,
records, and other pertinent information contained in my personal files. In the complaint I have
not wÉ1ere I fell and how I fellincluding my fractured wrist which was treated at Kings County
Hospital. I have also noted that at the time of the accident I was with my friend Chris Boland.
