PLEASE TAKE NOTICE that the undersigned hereby demands, pursuant to CPLR
3101(a) and 3120, that Plaintiff Yvette McClamb within thirty (30) days of the service of this
demand, provide Cross-Claimant-Defendant West New York Restoration of CT, Inc., with
authorizations which will permit them to examine Plaintiff Yvette McClamb's school records.
PLEASE TAKE FURTHER NOTICE that this authorization should permit Cross-
Claimant-Defendant West New York Restoration of CT, Inc. with access to information relating
to Plaintiff's grades, attendance, and medical history.
PLEASE TAKE FURTHER NOTICE that should Plaintiff Yvette McClamb willfully
fail to disclose the above information, Cross-Claimant-Third-Party-Defendant West New York
Restoration of CT, Inc. will actively pursue the remedies made available in Section 3126 of the
CPLR.

made for the appropriate relief to the New York State Supreme Court, New York County.
