IT IS HEREBY AGREED, CONSENTED AND STIPULATED to by the partes
herein that the following discovery remains outstanding and will be
completed as follows:
1. Deposition of NYCHA employee SUZETTE THOMAS shall be
completed on or before September 21, 2020 at 1 p.m. via Zoom by
mutual agreement of the parties;
2. Plaintiff is to provide defendant with any and all HIPAA-
compliant authorizations for any and all medical providers who have
treated the plaintiff for his injuries and conditions related to
this accident from the date of plaintiff’s deposition in 2019 to
the present time. These HIPAA-compliant authorization are to be
provided within forty five (45) days hereof. Also, plaintiff is
toprovide updated HIPAA-compliant authorizations for any
providerswho treated plaintiff prior to his deposition and
who havecontinued to provide treatment to the plaintiff from
the date ofhis deposition to the present time;

3. Plaintiff is to appear for an independent medical
examination (IME) within ninety (90) days of plaintiff’s service of
HIPAA-compliant authorizations upon the defendant. Defendant will
designate a medical doctor within thirty (30) days of receipt of
HIPAA-compliant authorizations from plaintiff’s counsel;
4. A continued Status Conference will be held on December 9,
2020, at 12:00 p.m. via teleconference, unless otherwise directed
by the Court prior to said date.
