The Plaintiff is a Federally-Chartered Credit Union, licensed to conduct business in New York, with a principal place of business at 90 East Bridge Street , Oswego, County of Oswego, State of NY. 2. Upon information and belief at alltimes hereinafter mentioned, the Defendant(s) is/are an individeal(s), with a residence at 338 County Route 11 Lot 92 West Monroe, NY 13167. 3. That there is due and owing to the Plaintiff by the Defendant(s), the sum of $6,021.92 which includes principal of $5,055.49, plus accrued but unpaid interest as of December 13, 2018 in the sum of $591.43, accumulated late charges and miscella.neous fees of $375.00, plus interest at the rate of 10.000% from December 13, 2018 by reason of the Defendant(s) default in the payment pursuant to the terms of a Loan and Agreernents and Disclosure Statement dated 8, 2015. Security January 4. That although payment has been dernanded of the Defendant(s) by the Plaintiff, such has been neglected and 'refásed and/or denied.
