(2) After the 61 day waiting period, call the Clerk of the Court at 817/884-2574, 817/884-2573 or 817/884-1581 after
10:00 a.m. to schedule your appointment to finalize your case. On the day you come in to finalize your case, you need to
be IN THE DISTRICT CLERK'S OFFICE {233RD CLERK'S AREA) ON TI-ill 3no FLOOR NO LATER THAN 8:00
A.M. OR 1:00 P .M•. If you are [ate, your appointment will have to be reset.
(3) Appropriate court attire is required (business casual). No shorts, tank tops, hats or caps. Children are not allowed
in the courtroom.
(4) You are responsible to ensure that all parties to the case receive personal service/notice of the case. All
requests for alternative service must be approved by the Court prior to notice being issued.
(5) It is YOUR responsibility to complete the below required paperwork:
• WAIVER- May be signed and notarized any day after the original petition is filed.
• DECREE OF DIVORCE or final order- You will need the original and two {2) copies (total of three (3)).
• DECREE OR ORDER WITH CHILDREN-You will need the original and three (3) copies ofa Wage
Withholding Order, along with a ROCS form (Record of Child Support). ROCS forms can be picked up in the
Child Support Office on the 2nd floor of this building. DO NOT BRING CHILDREN WITH YOU TO COURT.
• Information on Suit Affecting the Family Relationship (Austin form) (provided).
• Prepared written testimony for court (provided).
(6) If your address and/or telephone number change, you must notify the District Clerk's office in writing. You can
bring the information in at any time before your hearing date or on the day ofyour final hearing.
(7) In a suit involving children, each party shaU attend Family Court Services Orientation in the Assembly Room on
the 2nd floor of the Family Law Center, 200 East Weatherford Street, Fort Worth, Texas 76196 PRIOR to your scheduled
appointment Each party shaU file the completion certificate with the Court.
FAILURE TO APPEAR ON THE SCHEDULED COURT DATE WILL RESULT
