TO THE HONORABLE JUDGE OF SAID COURT:
PASQUALE PETER LaPOSTA ("Applicant") furnishes the following information to the
Court in support of this Application for the probate of the written Will of ANN MARIE LaPOSTA
(also known as Ann LaPosta and Ann Gallagher LaPosta), hereinafter called Decedent, as a
Muniment ofTitle:
1. Applicant is an individual interested in this Estate, being domiciled in and residing at
905 Live Oak Lane, Arlington, Tarrant County, Texas 76012. The last three digits ofthe Applicant's
Texas driver's license number are 139, and the last three digits of the Applicant's social security
number are 567. PASQUALE PETER LaPOSTA may be served with citation at 905 Live Oak Lane,
Arlington, Tarrant County, Texas 76012.
2. Decedent died on March 9, 2020, at Tarrant County, Texas, at the age of eighty-two
(82) years. Decedent's domicile at the time of her death was Tarrant County, Texas. The last three
digits of the Decedent's Texas driver's license number are 787, and the last three digits of the
Decedent's social security number are 252.
3. This Court has jurisdiction and venue because Decedent was domiciled in Texas and
had a fixed place ofresidence in this County on the date ofher death.

4. Decedent owned property described generally as real estate, cash, personal effects and
household goods of a probable value in excess of $100.00, including, but not limited to, real estate
described as:
Lot TEN (10)in Block EIGHT(8)ofTHE OAKS,5* Installment, an Addition
to the City of Arlington, Tarrant County, Texas, according to plat recorded in
Book 388-36, page 1, Deed Records of Tarrant County, Texas; whose street
address is 905 Live Oak Lane,Arlington,Tarrant County,Texas 76012.
5. Decedent left a valid written Will dated December 22, 2016, which was never
revoked and is filed herewith.
6. No children were bom to or adopted by Decedent after the date ofthe Will.
7. Decedent never had a dissolution of marriage after the Will was made.
8. Decedent did not apply for and receive Medicaid benefits on or after March 1,2005.
9. No state, governmental agency of the state, nor charitable organization is named by
the Will as a devisee.
10. The Will named PASQUALE PETER LaPOSTA as Independent Executor, whose
residence address is 905 Live Oak Lane, Arlington, Tarrant County,Texas 76012.
11. The Will was self-proved in the manner prescribed by law. The subscribing
witnesses to the Will are LINDA G.DOUGLAS and BRENDA D.SPROUSE.
12. Applicant has investigated the affairs of the Decedent and finds that to the best of
Applicant's knowledge and belief, there are no unpaid debts owing by the Estate of the Decedent,
exclusive of any debt secured by liens on real estate, and there is no necessity for administration of
such Estate.

WHEREFORE, Applicant prays that citation issue as required by law to all persons
interested in this Estate; that the Will be admitted to probate as a Muniment of Title only; that the
requirement for the filing ofan Affidavit of Fulfillment of Terms of Will be waived; and that all other
Orders be entered as the Court may deem proper.
