Plaintiff, SURF CONSULTANTS, INC. assuccessor in interest toChase Bank USA N.A.-First USABank
NA Card, by and through their undersigned counsel, moves this Court for an Order of Continuing
Garnishment against Salary or Wages on the pleadings in this cause and would state as follows:
1. Subsequent to the entry of Final Judgment against Defendant, DANIE HYPOLITE, a Writ of
Garnishment Against Salary or Wages was issued by the Court in the above-styled cause.
2. Pursuant to the Writ of Garnishment Against Salary or Wages, Garnishee, MOONLITE DINER
Il, INC., filed an Answer admitting that is indebted to the Defendant, DANIE HYPOLITE.
3. There still remains due and unpaid on the Final Judgment entered against the Defendant,
DANIE HYPOLITE, the sum of $8,416.86 plus post judgment costs and interest.
4, Defendant has not filed a response to the Writ of Garnishment in this cause.
WHEREFORE, Plaintiff requests that this Court enter an Order of Continuing Garnishment against
Salary or Wages in the above referenced cause.
