State of New York, County of New York, located at 60 Centre Street, New York, New
York 10007 on the 27th day of February, 2020 at 9:30 a.m., or as soon thereafter as
counsel can be heard, for an Order:

1. Pursuant to CPLR § 3215 granting plaintiff leave to enter a default
judgment against all of the defendants except for CHELSEA
MOBILITY, INC.;

complaint to add EASTERN MEDICAL PRACTICE, P.C., as
additional parties; and
3. For any such other relief that this Court deems just and proper.
PLEASE TAKE FURTHER NOTICE, that pursuant to CPLR § 2214(b), answer
papers, if any, must be served at least seven (7) days prior to the return date of this
motion.
