ADELE SMITH, individually, pursuant to Rule 1.350 of the Florida Rules of Civil Procedure, and hereby
request Defendant, OAKWOOD CENTER OF THE PALM BEACHES, INC. (hereinafter
OAKWOOD CENTER), to produce and permit the Plaintiffs to inspect and copy each of the following
documents on or before thirty (30) after service hereof (or within a shorter time period entered by the
Court) at the offices of the undersigned counsel.
Posted list of patients cleared for cafeteria privileges from the secured West Unit at
Oakwood Center of the Palm Beaches for the following time periods:(Note that the last
names of patients on the list other than Ralph Smith’s can be redacted).

b. On the day prior to the incident, January 26, 2004
c. For the week prior to the incident, January 19, 2004 through January 23, 2004.
