                            Itis hereby stipulated by and between the attorneys for Plaintiff, HERTZ VEHICLES,

                  LLC, (“Hertz”) and Defendant, MIDDLE VILLAGE DIAGNOSTIC IMAGING, P.C. that:

                           1       MIDDLE VILLAGE DIAGNOSTIC IMAGING, P.C. releases and waives any

                  and all No-Fault claims they may ever have asserted, arising out of the July 20, 2014,

                  collision referenced in the complaint (also known as claim number 02-2014-18327).

                           2       In consideration of this release, the action is hereby discontinued without

                  prejudice against MIDDLE VILLAGE DIAGNOSTIC IMAGING, P.C. only.

                           3.     MIDDLE VILLAGE DIAGNOSTIC IMAGING, P.C. shall discontinue any and

all lawsuits or arbitrations seeking to collect no-fault benefits for the above referenced

claims.

          4      A facsimile of signatures shall be deemed as originals.
