1. The Plaintiff is a National Banking Association, licensed to conduct business in New York, with a
principal place of business at 45-49 Court Street , Canton, County of St. Lawrence, State of NY.
2. Upon information and belief, the Defendant(s) is/are an individual(s), with a residence at 14023 Canada
St. Red Creek, NY 13143.
3. That there is due and owing to the Plaintiff by each Defendant, the sum of $6,797.41, plus interest at the
rate of 10.590% from 09/09/15, plus reasonable attorneys fees to the extent provided by the contract and
allowed by the laws of the State of New York, by reason of Defendants default in payment for money lent by
the Plaintiff to the Defendant.
4. That although payment has been demanded of the Defendant(s) by the Plaintiff, such has been neglected
and refused.
WHEREFORE, Plaintiff demands judgment against the Defendant(s) in the sum of $6,797.41, together with
interest at the rate of 10.590% from 09/09/15, reasonable attorneys fees, if provided by contract and allowed
