adjourned date to give testimony in this action, which relates to an incident that occurred on October
15,2015. A copy of the police report (Incident No.: 2015-00023587) pertaining to this incident is
annexed to this subpoena.
WE FURTHER COMMAND YOU that said persons to be examined are required to
produce all log notes or memo pad, all records, and papers in their custody and possession
relating to the accident that occurred on October 15, 2015 as per the attached police report.
