The Plaintiff(s) states he/she has a claim against the Defendant(s) in the amount of §. 5,000.00 lus court costs.
The claim arose on or about lune Sth 202. as a result of the following events:
A loan in the amount of $11,000 was extended to Mrs. Keisha Haynes of Purpose Driven Academy/Vision Center
on August 29, 2023. It was mutually agreed that Mrs. Haynes would deduct $710.00 monthly from the loan amount to cover
the tuition of Oriah Perkins. As of June 6, 2024, Oriah Perkins is no longer enrolled at Purpose Driven Academy/Vision Center.
It is alleged that Mrs. Haynes created a hostile environment, rendering it unsafe for Oriah Perkins &
family members to continue attending/visiting Purpose Driven Academy/Vision Center. | am seeking
reimbursement for the remaining loan amount allocated towards Oriah's tuition from September 2023 to May 2024,
lam requesting an additional $390 in punitive damages.
The Plaintiff(s) states that the allegations in this petition are true and correct to the best of his/her knowledge, that he/she
is not an assignee of this claim, and that he/she has not filed more than twelve other claims inj Missouri smail claims
courts during the current calendar year. Plaintiff(s) understands that should he/she be successful in this action and obtain
a judgment, and if defendant(s) does not appeal by Trial de Novo and this judgment becomes ‘final, the plaintiff(s) is
barred from commencing another action involving the same parties and issues. Plaintiff(s) understands that he/she is
hereby waivingapp lay to jury trial on these issues in the Small Claims Court.
