                       CONVICTION RECORDS UNDER IC §35—38-9-4

         COMES NOW, Petitioner, Thea A. Hullinger, by counsel, and does file this Petition

Seal and Expunge Conviction Records under the provisions of IC § 35-38-9-4. Petitioner

respectfully moves this Court to expunge Petitioner's records filed under cause number

01D01-1008-FD-000082 and order to be sealed Petitioner's arrest and conviction records

related to this cause number.

         Petitioner afirms under the penalties of perjury:

     l. Petitioner's full name is Thea A. Hullinger.

    2.   Petitioner has no other legal names or aliases by which Petitioner has been known.

    3.   Petitioner's date of birth is October 15, 1991.


    4. Petitioner's Social Security Number is             --4036.
    5.   Petitioner's Driver's License Number is 3380-09-8745, and is an Indiana License.

    6.   To the best of her knowledge, Petitioner's addresses fiom the date of the ofl'ense listed

         in this Verified Petition to the date of signing this Verified Petition are/ have been (most

         current address listed last):
            a. 1122 Lewis Street, Decatur, IN 46733.
            b. 3050 West Shady Side Road, Angola, IN 46703.
            c. 5727 Kinlock Place, Fort Wayne, IN 46835.

    7. Pursuant to this Petition, Petitioner is seeking relief for the following matter:

    8. Petitioner has no other convictions.

    9. Petitioner certifies that at least eight (8) years have elapsed since the date of her

        conviction and that she has no other criminal convictions.

    10. At the time of filing this petition, Petitioner has no knowledge of any current

        criminal charges or criminal investigations pending against Petitioner in any

        jurisdiction and Petitioner is not currently participating in a pretrial diversion

        program.

    11. Petitioner has successfully paid all fines, fees, and court costs, and satisfied any

        restitution obligation imposed on the person as part of the sentence if applicable for

        any and all criminal arrest(s) and or charges(s) contained within this petition for

        which a sealing and or expungement is sought.

    12. Petitioner will serve a copy of this Verified Petition for Expungement upon the

        Prosecuting Attorney in accordance with the Indiana Rules of Trial Procedure.

    WHEREFORE, Petitioner respectfully requests this Court GRANT this Petition and

ORDER the conviction records contained in the Court’s files, in the files of the Indiana

Department of Corrections, in the files of the Indiana Bureau of Motor Vehicles, in the

files of any other person who provided treatment or services to Petitioner under a court
order; and, any records in the central repository for criminal history information

maintained by the Indiana State Police, be EXPUNGED AND SEALED, and for all

other just and proper relief.
