LLC (“Defendants”), in the above captioned matter, at McCabe & Mack LLP, 63 Washington
Street, Poughkeepsie, New York 12601 on Tuesday, November 28, 2017 at 10:00 a.m., to
give testimony and give evidence in the above-captioned matter.
The subject matter of your testimony is in regard to an alleged accident by the Plaintiff,
Hannah Buckstine at Jordan’s of New Paltz, LLC, 52 Main Street, New Paltz, New York on
October 25, 2014.
Failure to comply with this subpoena is punishable as a contempt of Court and shall make
you liable to the person on whose behalf this subpoena was issued for a penalty not to exceed
fifty ($50) dollars and all damages sustained by reason of your failure to appear.
