Comes now Plaintiff, by and through counsel, and for its Complaint states herein:
1. The Defendant is liable to the Plaintiff under the Credit Card Account Agreement, a copy of which is
attached hereto as Exhibit A.
2. Said obligation is past due, and Defendant owes the principal amount of $4,882.65 plus interest from
the date of judgment at the statutory rate. STATEMENT is attached hereto as Exhibit B. AFFIDAVIT is
attached hereto as Exhibit C.
Wherefore, the Plaintiff prays for judgment against the Defendant for the sum set forth above plus interest at the
statutory rate from the date of judgment, and costs.
