Defendant requested a credit account ("Account") be opened, and an Account was opened on or about 04/30/2021 for the
Defendant's use. The previous or original creditor is Credit One Bank, N.A./. The account was sold. Plaintiff acquired and
is now the owner of the account which is the subject matter of this action. The original Account number (now closed) is
XXXXXXXXXXX9809.
Defendant is indebted to Plaintiff on the overdue unpaid balance owing on the Account of $2,189.03 ("Account Balance")
Defendant failed to pay the Account Balance. Defendant is indebted to Plaintiff in the amount of $2,189.03, on an account
stated on or about 05/15/2022. The last payment was made on or about 11/13/2021.
3. I understand that if I do not come to court on my hearing date, my case will be dismissed and I may have to pay money to
the Defendant on any counterclaim that has been filed.
