AFFIDAVIT TO PROVE UP AGREED FINAL DECREE OF DIVORCE
Neal Andrew Porter appeared in person before me today and stated under oath:
"My name is Neal Andrew Porter. I am above the age of eighteen years, and I am fully
competent to make this affidavit. The facts stated in this affidavit are within my personal
knowledge and are true and correct.
"T am the Plaintiff in this case. Lynda Simon Lansford is my spouse.
"At the time I filed this suit for divorce, I had been a domiciliary of Texas for the
preceding six months and a resident of this county for the preceding ninety days.
"My spouse and I were married on or about May 15, 2020 and ceased to live together as
spouses on or about February 24, 2024.
"The marriage has become insupportable because of discord or conflict of personalities
between my spouse and me that destroys the legitimate ends of the marriage relationship and
prevents any reasonable expectation of reconciliation.
"No children were born or adopted during this marriage and none is expected. The Wife
did not have a child with another man while married to the Husband.
"My spouse and I have reached an agreement regarding the division of the community
estate, both assets and liabilities of the marriage, which is set forth in the proposed Final Decree
of Divorce. The proposed division of the community estate is a just and right division of that

"My spouse and I have signed the proposed Final Decree of Divorce. I recognize my
spouse's signature where it appears on these documents. I respectfully ask the Court to grant this
divorce and approve all the agreements my spouse and I have made in them.

FAMILY CASES - Case Captions and Styles MI
The following Local Administrative Rule was adopted by the consent of thd undersigned District
and County Court at Law Judges with regard to the identification of parties and children in
actions brought pursuant to TitlesI, II, IV and V of the Texas Family Code, excluding adoption
and Chapter 33 cases.
LOCAL RULE
= Texas Family Code Sections 6.401 and 102.008 require the full names of the parties and the
names of any minor children to be included in the caption/style of the case, the District Clerk is
ordered to refuse to accept for filing any pleading that does not contain the required
identification and/or contains only the initials of the parties and/or the children in the style of the
case. This rule does not apply to the styling/caption of adoption cases or cases pursuant to
"apter 33 of the Texas Family Code. This rule is effective as of April 1, 2016.
