YOU ARE NOW HEREBY ORDERED to comply with the provisions of the Labor Law by remitting to
the Commissioner of Labor the total sum as indicated above, for any wages due, plus interest and/or
civil penalties assessed, to be disbursed according to law.
Checks and money orders should be made payable to the Commissioner
of Labor and sent to the New York State Department of Labor, Division of
Labor Standards, State Office BIdg. Campus, BIdg. 12, Rm 266A, Albany,
NY 12240.
*If you are aggrieved by this Order*, you may appeal within 60 days
from the date issued to the Industrial Board of Appeals as provided by
Section 101 of the Labor Law. Your appeal should be addressed to the
Industrial Board of Appeals, Harriman State Office Campus, BIdg. 12,
Room 183, Albany, NY 12440.

IT IS FURTHER ORDERED that, in the event that you fail to remit payment of the wages due, and
the interest, liquidated damages and civil penalty(ies) assessed within 60 days of the date of this
Order, and no petition has been filed with the Industrial Board of Appeals for review of this
determination, this Order may be filed with the County Clerk where you, the Employer(s), resides or
has a place of business, and the Clerk shall without further notice enter judgment pursuant to
Section 218 and Section 219 of the New York State Labor Law in the total amount due, as indicated
above, with the additional interest at the stated rate of interest from the date hereof until payment. At
the discretion of the Commissioner, the Commissioner shall have full authority to provide for inclusion
of an automatic fifteen percent additional amount of damages to come due and owing upon expiration
of ninety days from the Order to Comply becoming final.
