The undersigned states: that s/he is one of the attorneys for Plaintiff in the above-entitled
action; that the Summons and Complaint in said action have been duly served on Defendant(s)
and said Summons and Complaint according to the proof of said service duly filed in the office
of the Clerk of Court; that an Answer or other pleading has been received by or served upon
Plaintiff through its attorney; a true and correct copy of which is attached hereto as Exhibit A.
Declarant further states that the Answer of Defendant(s) does not deny liability of the
debt referenced in Plaintiff's Complaint. Accordingly, the Court upon consideration of the
Plaintiff's Complaint and the Answer of Defendant(s) should enter Judgment on the Pleadings
pursuant to Minnesota Rules of Civil Procedure, Rule 12.03.
Declarant further states that to the best of his/her knowledge, information and belief, the
full name and address of Defendant(s) are Mattu Saidu 235 NATHAN LN N APT 231
PLYMOUTH MN 55441-6331 and that Defendant's occupation is unknown.
Declarant further states that pursuant to a search of the Department of Defense
Manpower Data Center the Defendant(s) above named, is not now in the military service of the
United States and that this affidavit is made in compliance with the Servicemembers Civil Relief
Act.
Declarant further states that he/she has read the Complaint in this action and knows the
contents thereof, and that the Complaint alleges that there is now due by Defendant(s) to Plaintiff
on the debt set forth the amount of $4,857.42.

have all those kids and no one to help you. really want to get the issue

apply for unemployment but was not qualified do to some reason

don't know, but like l said before I can only afford $50 ~75 dollar a

even afford food for my kids to eat. just get back on my feet 6 months
