Comes now the Plaintiff, ARVEST BANK, and for its cause of action against the
Defendant{s) alleges and states as follows:
1 The Defendant{s) herein is a resident of SCOTT County, Arkansas and this
Court has jurisdiction of the parties and the subject matter herein.
2 The underlying obligations owed by the Defendant(s) to the Plaintiff result
from charges made by the Defendant(s) on a ARVEST BANK credit account. The account
was opened on May 23, 2020.
3 Demand has been made upon the Defendant(s} and payment has not been
forthcoming.
4 Defendant(s), Stansel F Donna, is indebted to Plaintiff for the sum of
$1,571.77, Attached hereto and incorporated herein by reference is an affidavit executed on
behalf of Plaintiff. A copy of the cardmember agreement (the written instrument(s) upon
which the Plaintiffs claim is based) is attached hereto and incorporated herein by
Teference.
5 Plaintiff is the lawful hoider of the Account and Defendant(s) has failed,
refused and neglected to pay the same after due and proper demand thereof.

6 ARVEST BANK is entitled as a matter of law to a judgment in its favor and
against Defendant(s} for the total amount due, such being $1,571.77, with a reasonable
attorney's fee, post-judgment interest at the maximum rate allowed by Arkansas law, and

WHEREFORE, PREMISES CONSIDERED, Piaintiff, ARVEST BANK, prays for judgment
against the Defendant(s), Stansel F Donna, in the sum of $1,571.77, with a reasonable
attorney's fee, for post judgment interest at the maximum rate allowed by Arkansas law,
with the costs of this action, and all other relief to which the Plaintiff may be entitled.

The undersigned, being duly swom, states the following under oath:
1 I am over eighteen (18) years of age and am competent to make this affidavit, I
am an agent of Arvest Bank (herein tho “Plaintifi"), My job title/relationship to Plaintiff is

responsibilities.
2, Tam familiar with the books and records ef Plaintiff, the manner and method by
which Plaintiff maintains said books and recordsin the regular course of its business, and
computer records of defaulted accounts, including the account described in this affidavit, These
books and records are made in the course of regularly conducted business activity; at or near the
