Plaintiff's counsel, being duly sworn upon oath deposes and says as follows:

1.     That she is one of the attorneys for the judgment creditor herein, PAC Western Financial,

       LLC, and that she makes this affidavit in compliance with the Uniform Enforcement of

       Foreign Judgments Act as promulgated in Minnesota Statute Chapter 548.

2.     That attached hereto is an authenticated copy of the judgment entered in the Utah Court

       of Salt Lake County, in the amount of $41,357.21.

3.     That this judgment has not been filed in any other county in the State of Minnesota.

                                     AFFIDAVIT OF IDENTITY

4.     The full name of Defendants are Health Professional Network Inc dba Health

       Professional Network and Martha Sinoe. Health Professional Network Inc’s last known

       address is 6040 Earl Brown Dr 101, Brooklyn Center MN 55430. Martha Sinoe’s last

       known address is 9828 Greenspruce Ave N, Brooklyn Park MN 55443. Defendants’

       places of employment are unverified.

5.    Plaintiff believes that Defendant is not currently in military service on behalf of the

      United States. This Affidavit is therefore in compliance with the Servicemembers Civil

      Relief Act.
