AFFIDAVIT AS TO DEATH AND APPLICATION FOR PROBATE OF WILL
SHANDA G. ALLEN, being first duly sworn, deposes and says that Edwards
James “E.J.” Hampton who resided at 296 CR 134, Wasola, Ozark County, Missouri,
died on the 23 day of February, 2024, at Wasola, Missouri.
That decedent left an instrument in writing dated January 18'", 2019, purporting
to be his Last Will and Testament. The subscribing witnesses to said instrument, given
here in the order of the date of the execution of said instrument, are Christopher
Swatash and Shanda Allen.
WHEREFORE, Petitioner prays that after said instrument has been duly proved
that the same be admitted to probate and recorded as the Last Will and Testament of
said decedent.
