states as follows:

        I.   I am  Compliance Associate of Plaintiff above-named, whose office is located at'1800
ROUTE 34 N, WALL-NJ 077 I9,          and that I am familiar with the day-to-day business operations
of Plaintiff and its record- keeping practices. In preparing this affidavit, I have relied on my own
:lcnowledge, as well as information contained 1n the records kept by Plaintiff. This Affidavit IS
 made in support of.-           rcqucst for default

        2.    Plaintiff IS the holder of certain accounts of INDIGO MASTERCARD ISSUED
BY CELTIC BANK SERVICED BY GENESIS FS CARD SERVICES                      INC, including account
of Defendant which IS the subject of this matter; attached hereto as Exhibit "A" is a true and
correct copy of the applicable Bill of Sale.

     .3.   That Defendant applied for credit trom INDIGO MASTERCARD ISSUED BY
CELTIC BANK SERVICED BY GENESIS FS CARD SERVICES INC, and INDIGO
MASTERCARD ISSUED BY CELTIC BANI< SERVICED BY GENESIS FS CARD
SERVICES INCaccepted Defendant's application, and issued to Defendant a credit card having
account number ending In *****_*******4555; attached hereto as Exhibit "B"

correct copy of the CreditAgreement Terms.

         4.     'lhat Defendant owes Plaintiff for the purchase of goods, wares, and services, on
or be fore February 8, 2022, the sum of $1, 618. 77 as shown 1n the attached Billing Statements
labeled as Exhibit "C"                                                         ~




        5.      Although Plaintiff made demands upon Defendant for the payment of the
aforesaid sum, Defendant has failed and neglected to pay same.
