Robert A. Weis, Esq., an attorney duly admitted to practice law in the Courts of the State
of New York, affirms the following statements to be true under the penalties of perjury:
1. I am associated with the Law Firm of William G. Sayegh, P.C., the attorneys for
the Petitioner, JENNIFER FORDHAM-EBERZ, in connection with the within Petition for a Name
Change for a Minor and as such, I am fully familiar with the facts and circumstances herein. I
make this affirmation in support of the Petitioner's Petition pursuant to New York State Civil
Rights Law Section 60 for a name change and for the sealing of the record in this matter pursuant
to New York State Civil Rights Law Section 64-a.
2. The Petition seeks to change the name of the minor child, ASHLEE ROSE
FORDHAM to ASH R. FORDHAM.
3. As set forth in Exhibit C of the Verified Petition, James J. Lazier, the minor's father
(who currently resides in Florida) has reviewed the Petition and consents to the Court's issuance

of an Order granting the Petition. (The affidavit of James J. Lazier, dated the day of February,
is annexed to the Petition as Exhibit C).
4. As set forth in Exhibit B of the Verified Petition, ASHLEE ROSE FORDHAM, the
minor child, has reviewed the Petition and having sought the filing of the Petition for a name

change from his parents, consents to the granting of the reliefin the Petition and requests the Court
to issue of an Order granting the Petition.
5. The Court is presented with both the parents and the child agreeing that it would be
in the best interests of the child, who is currently seventeen years old and will be eighteen years

6. In accordance with New York State Civil Rights Law Section 64-a, Petitioner also
requests that since the name change request is associated with a gender transition, that the Court
record in this matter be sealed, to be opened only by order of the Court for good cause shown or
at the request ofPetitioner or by ASHLEE ROSE FORDHAM, once ASHLEE ROSE FORDHAM
has reached the age of eighteen.
7. No prejudice to the rights of any individual, living or dead, could ensue from the
granting of the Order.
8. No previous application has been made for the relief sought herein.
WHEREFORE, your affirmant respectfully requested that the Court issue au Order
pursuant to New York State Civil Rights Law Section 60, permitting the Petitioner ASHLEE
ROSE FORDHAM to assume the name of ASH R. FORDHAM, and pursuant to New York State
Civil Rights Law Section 64-a sealing the record in this matter, together with such other further
