4. I have read the foregoing Verified Petition dated February 12th, 2024, requesting the Court
to issue an Order changing my child's name from ASHLEE ROSE FORDHAM to ASH R.
FORDHAM and I consent to the change.
9. I respectfully request that the Court accepts the Petition for the reasons set forth above and
so the name my minor child desires to be known by will match his legal identification.
