1. My present name is Gurpal Singh.
2. I wish to change my name to Gurpal Singh Bhatia.
3. My present address is 26 Stepping Stone Crescent, Dix Hills, New York, 11746, in
the County of Suffolk.
4. I was born on October 22, 1991 and I am now 32 years of age.
5. I was born in New Delhi, India.
6. Proof of birth is submitted with this name change application.
7. I am married.
8. I am not currently confined as an inmate in a correctional facility or currently under
the supervision of the State Division of Parole, or a County Probation Department as
a result for a conviction for a violent felony offense or other felony as specified in
Civil Rights Law.
9. I have never been convicted of a misdemeanor or felony.
10.1 have never declared bankruptcy.
11.There are no judgments or liens of record against me.
12.I am not a party to any legal actions, lawsuits, or legal proceedings.

would like to change my name. While Singh represents the Sikh culture, the correct
conventions include a family name, by adding Bhatia I would be able to correctly
have a name according to my religion.
16.1 have not made a previous application to change my name in this or any other court.
WHEREFORE, Petitioner respectfully asks for an order permitting the petitioner
to assume the name Gurpal Singh Bhatia.
