At all times mentioned herein, plaintiff and judgment creditor was and remains a public welfare department located in the County of Lewis and State of New York. 2. On information and belief, defendant and judgment debtor resides in the County of Lewis and State of New York. 3. On or about May 23, 2014, judgment by default, Index No.: CA2013-000006 was duly entered in favor of this plaintiff and against this defendant for the sum of Three thousand eight hundred sixteen dollars and 09/100 ($3,816.09) in the Lewis County Clerk's Office. A copy of said judgment is annexed hereto and marked Exhibit "A". 4. To date, no payments have been received, leaving the judgment balance remaining due of $3816.09 plus interest. 5. Said judgment was duly docketed on May 23, 2014, in the Lewis County Clerk's Office, more than nine years prior to the commencement of this renewal action. 6. This action is brought under CPLR 5014(1). WHEREFORE plaintiff demands the entry of a renewal of the existing judgment in favor of the plaintiff and against the defendant for the sum remaining due on said judgment with interest, together with costs and disbursements of this action and such other, further and different relief as to this court may seem just.
