01. Lucia DiEmidio, being duly sworn, deposes and says that she is over the age of 18 years,
is not a party to the above action, and resides in Suffolk County, State of New York. I am
employed by Nationwide Court Services, Inc. (hereinafter NCS) as a researcher, I make
this affidavit based upon my personal review and inquiry of business records prepared
and maintained in the regular course of business of NCS.
02. NCS received from Sheldon May & Associates, P.C., the attorneys for Plaintiff in the
above-named action, a request to effectuate in-hand service upon ROBERT PURCELL.
03. Your deponent conducted an advanced online search for ROBERT PURCELL. Said
search returned two listings for ROBERT PURCELL at the following addresses (Exhibit
A):

" P.O. Box 1110, Woodridge, NY 12789
04. Your deponent conducted an online license search of the NYS Department of Motor Ve-
hicles website for ROBERT PURCELL, using the defendant's name and date of birth.
Said search returned a record for the defendant at 20 Nevins Street, Ellenville, NY 12428.

05. Your deponent conducted an online voter registration search of the NYS Board of Elec-
tions using the defendants name, date of birth and county of Ulster. Said search returned
a record for ROBERT PURCELL at 20 Nevins Street, Ellenville, NY 12428. (Exhibit C).
06. Your deponent conducted an advanced online search for a place of business for ROBERT
PURCELL. Said search yielded no record for the defendant. (Exhibit D).

08. Your deponent conducted an online inmate search of the NYS Department of Corrections
and Community Supervision for ROBERT PURCELL. Said search returned no record for
the defendant. (Exhibit F).
09. Your deponent conducted online inmate search of the NYC Department of Correction
for ROBERT PURCELL. Said search returned no record for the defendant. (Exhibit G).
10. Your deponent conducted an online inmate search of the Federal Bureau of Prisons for
ROBERT PURCELL. Said search returned no record for the defendant. (Exhibit H).
1I. Your deponent conducted an online search of the Department of Defense Manpower Data
Center for ROBERT PURCELL. Said search confirmed that the defendant is not a
member of the uniformed services. (Exhibit I).
12. On December 1, 2023, NCS by its agent(s) or employees made a formal request upon the
United States Post Office in Ellenville, NY 12428 for ROBERT PURCELL at 20 Nevins
vacant."

door."

search provided no new information for the defendant. (Exhibit N).
17. On December 22, 2023, your deponent's agent, Olivia Charpentier, conducted a search of
the records of the Surrogate's Court, County of Ulster, which revealed that ROBERT
PURCELL had no filings or proceedings with said court. (Exhibit O).

19. On December 22, 2023, your deponent attempted to contact ROBERT PURCELL at 201-
941-0375. The phone number is not in service.
20. On January 11, 2024, your deponent's agent, Tamasha Manigault, attempted to serve
ROBERT PURCELL at 357 Mountaindale Rd, Apt 43, Woodridge, NY 12789 with the
following result, "This address is a condo. Director in leasing office stated address
down."
