IT IS HEREBY STIPULATED BY AND BETWEEN THE PARTIES that the SUM
arbitration concerning Jack Dominie, demanded February 27, 2023, is temporarily stayed in order
for the parties to complete all necessary and relevant discovery;
IT IS FURTHER STIPULATED THAT DISCOVERY WILL PROCEED AS
FOLLOWS:
1. Counsel for Respondent will provide duly executed unrestricted authorizations for all
relevant providers including respondent's primary care physician and pharmacy by
April 25, 2023;
2. Counsel for Respondent will exchange all relevant documents from the underlying
action, including but not limited to the Bill of Particulars, discovery responses, expert
reports, deposition transcripts and IME reports by April 25, 2023;
3. The Examination Under Oath of the Respondent is to be completed by August 15, 2023;
and
4. The IME ofthe respondent is to be conducted within sixty (60) days of the Examination
Under Oath and the report exchanged within forty-five (45) days thereafter.
