The Defendant owes me $3,684.85, plus filing fees and costs in the amount of $80.00, so my total
claim is for $3,764.85 (amount Defendant owes plus filing fees and costs). I have a claim for this
amount because the following happened (briefly describe):
Defendant’s account formerly with First Bank & Trust was opened on July 07, 2021. Defendant has
received billings for the amount due, has failed to object to the billings and/or has failed, refused or
neglected to pay the balance due and owing Plaintiff. Plaintiff is the assignee of Defendant’s First Bank
& Trust account. WHEREFORE, Plaintiff requests a judgment in its favor and against Defendant for
total damages in the amount of $3,684.85, plus requested filing fees and costs of $80.00, for a total of
$3,764.85 plus costs and interest on any judgment received allowable by statute and other such relief as
the court may deem appropriate.
