                   BEFORE ME, the undersigned Notary Public, personally appeared
                       attorney for Plaintiff herein, who being first duly swom, deposes and says:


                   1           Affiant is the attorney of record for the Plaintiff.
                   2      Diligent search and inquiry have been made to discover the residence, post office
           address or mailing address of the Defendant(s), NICOLE LEA COTTERMAN who are not
           known to be dead or alive, whether said unknown parties claim as heirs, devisees, grantees,
           assignees, lienors, creditors, trustees, spouse(s), or other claimants, and as particularly as is
           known to Affiant said Defendant(s) either cannot be located after diligent search and inquiry, or

that they conceal themselves so that process cannot be personally served upon and all unknown
parties claiming by, through, under or against the Defendant(s), NICOLE LEA COTTERMAN,
who are not known to be dead or alive, whether said unknown parties claim as heirs, devisees,
grantees, assignees, lienors, creditors, trustees, spouse(s), or other claimants, and A ffiant verily
believes that there is no person in the State upon whom service of process would bind said
Defendant(s).
       3             Affiant has been unable to determine the residence of and all unknown parties
claiming by, through, under or against the Defendant(s), NICOLE LEA COTTERMAN, who are

not known to be dead or alive, whether said unknown parties claim as heirs, devisees, grantees,
assignees, lienors, creditors, trustees, spouse(s), or other claimants, as evidenced by the
Affidavit(s) of Due and Diligent Search attached hereto as Exhibit "A".
       4.           The last known mailing address of Defendant(s), NICOLE LEA COTTERMAN is
as follows: 539 TENNEY AVE, CAMPBELL, OH 44405,
       5            Defendant(s), NICOLE LEA COTTERMAN are over eighteen (18) years of age.
