The Division of Employment Security, State of Missouri, hereby certifies that, on or before the 15th day of
November, 2023 pursuant to the provisions of the Missouri Employment Security Law, Chapter 288, of the Missouri
Revised Statutes (RSMo), an Order of Assessment of Contributions, Interest, and Penalties, Number(s) 2023180529 was
made against JOSH STEARNS, an employer, that resides in or has a place of business in, or has property in theCounty
of Stoddard, State of Missouri.
It is further certified that notice of the said order was served on above, as required by law.
It is further certified that pursuant to Section 288.160, RSMo, that the Order of Assessment against the above-referred
employer is final.
It is further certified to you, as Clerk of said Circuit Court, that the amounts of the said contributions, interest, and
penalties assessed and now due and owing the State of Missouri and more particularly the Division of Employment Security
for the period January 1, 2023 to June 30, 2023 are $147.35 contributions, $6.16 interest, and $400.00 penalties, being a
total of $553.51, together with all costs, including the cost of filing this certificate and of any subsequent proceedings, and
also interest pursuant to Section 288.150, RSMo, as amended, per month or part of a month until payment is received by
the Division of Employment Security and any additional penalty pursuant to Section 288.160, RSMo, as amended.
