as an authorized agent of Affinity Plus Federal Credit Union.
2 That Affiant has personal knowledge of the matters herein stated due to the fact that
Affiant is familiar with Defendants in that s/he has monitored and supervised the
account of the Defendants, which bears an account number ending in 8529.
That Affiant is the custodian of Plaintiff s records for Defendants' account.
That according to Plaintiff's records, Defendants were issued a credit card account
ending in the numbers 8529 ("Account") on September 05, 2018.
5 That Defendants used the Account to make purchases and/or cash advances.
6 That according to Plaintiff' s records, Defendants were required to make minimum
monthly payments.
That according to Plaintiff" s records, Defendants defaulted on the Account by failing to
make all of the required payments.

8. That the last payment made was on February 18, 2023, in the amount of $0.75.
9. That Exhibit A attached hereto is a true and correct copy of the billing statements
attesting to the Account 8529.
10. That according to Plaintiff's records, the current principal balance owed by Defendants on
the Account is $5,482.81.
