the Plaintiffs Complaint. In your Answer you must state whether
you agree or disagree with each
paragraph of the Complaint. If you believe the Plaintiff should not be given everything asked for in the
Complaint, you must say so in your Answer.
4. YOU WILL LOSE YOUR CASE IF YOU DO NOT SEND A WRITTEN
RESPONSE TO THE COMPLAINT TO THE PERSON WHO SIGNED THIS SUMMONS. If
you do not Answer within 21 days, you will lose this case. You will not get to tell your side of the story,
and the Court may decide against you and award the Plaintiff
everything asked for in the Complaint. If you
do not want to contest the claims stated in the Complaint, you do not need to

Defendant(s) owe(s) Plaintiff $5,482.81 for goods and services sold and delivered to
Defendant(s) and/or by receiving a cash advance or balance transfer. Said
purchases were made on
Defendant('s') charge account bearing account number XXXXXXXXXXXX8529 which
was/were
issued to Defendant(s) by Affinity Plus Federal Credit Union.
II.
Defendant(s) is/are in default for failing to make the required
payments on the charge account.
III.
In addition to the balance described above,
Defendant(s) also owe(s) Plaintiff interest in the
amount of $76.60, representing interest on the balance at the
rate of 6.00% per annum from
February 22,
2023, through May 30, 2023, plus continued interest on the balance at the rate of
6.00%.
