PERSONALLY APPEARED before the undersigned officer, duly authorized to
administer oaths, Jennie Frank , who states under oath
as follows:
1. I am over 18 years old and competent to testify to the matters
set forth herein. I am the Vice President,Loss Mitigation of
State Employees Federal Credit Union (hereinafter "SEFCU").
2. The scope of my job responsibilities includes the performance of
collection and recovery services for SEFCU. In the performance
of my duties, I am familiar with the manner and method by which
SEFCU creates and maintains its normal books and records,
including computer records of its accounts, monthly statement
mailings and payments applied. I have access and training on
the system of record utilized by SEFCU to enter and maintain loan
account records and documentation concerning the Defendant's loan
for the Plaintiff.
3. The records are made: (1) at or near the time the events they
purport to describe occurred, by a person with knowledge of
acts and events; or, (:2) by a computer or other similar digital
means, which contemporaneously records an event as it occurs. The
systems of record are accurate and reliable based on my training
and use of them for the maintenance of loan records.
4. From the books and records of the corporation kept in the regular
course of its business, the entries in them being made in the
regular course of business, I have full knowledge of the cause of
action upon which Plaintiff issuing, and make this Affidavit in
support of the Plaintiff's motion for Judgment.
5. Defendant, entered into a credit agreement with SEFCU. A copy of
the agreement is annexed hereto as Exhibit A.
6. The Defendant has failed to make payment despite demand. A true
and correct copy of the accounting history of the loan detailing
the amount due is annexed hereto as Exhibit B.
7. The Defendant is not an infant nor is Defendant on active duty in
any of the Armed Forces in the United States of America.

8. On behalf of Plaintiff, I request judgment in the sum of $8745.21
plus interest at 6.99% as of 11/24/20 and for other relief this
Court deems just and proper.
9. I declare under the penalty of perjury that the foregoing is true
and correct and if called as a witness I would competently testify
under oath, thereto.
