2021, deponent provided the additional notice required by CPLR Sec.3215 and the attached notice pursuant to
the Fair Debt Collection Practice Act ("FDCPA notice") on Defendant, Steven A. Greene, by mailing a copy of
the Summons and Complaint herein by first class mail, postpaid to said defendant at:

( ) defendant's place of employment inasmuch as
( ) the mailing to defendant's last known residence was returned by the Post

and did not indicate on the outside thereof by return address or otherwise, that it was from any
attorney or concerned an alleged debt.
Deponent further states that the mailing referred to herein was not returned as undeliverable by the post
office prior to the entry of the default judgment herein.
