3. All Franciscan Handmaids of the Most Pure Heart of Mary, Inc. expenses
relating to premises known as 63 Bayside Lane, Staten Island, New York arc paid by check.
4. I write all Franciscan Handmaids of the Most Pure Heart of Mary, Inc. checks
relating to premises known as 63 Bayside Lane, Staten Island New York.
5. At no time did I write a Franciscan Handmaids of the Most Pure Heart of
Mary, Inc. check for the installation, repair, or alteration of the sidewalk located on the
Woodvale Avenue side of premises known as 63 Bayside Lane, Staten Island, New York.
6. At no time prior to January 15, 2020 did the Franciscan Handmaids of the
Most Pure Heart of Mary, Inc. install, repair, or in any way alter the sidewalk located on the
Woodvale Avenue side of premises known as 63 Bayside Lane, Staten Island, New York.

Most Pure Heart of Mary, Inc. hire a contractor or other person to install, repair, or in any way
alter the sidewalk located on the Woodvale Avenue side of premises known as 63 Bayside
Lane, Staten Island, New York.
8. At no time prior to January 15, 2020 did a volunteer or any person, on behalf
of the Franciscan Handmaids of the Most Pure Heart of Mary, Inc. install, repair, or in any
way alter the sidewalk located on the Woodvale Avenue side of premises known as 63
Bayside Lane, Staten Island, New York.
