criminal convictions pursuant to Indiana Code § 35-38-9. In support of this petition, Petitioner
affirms under penalties for perjury:
1. My full legal name is Brandy Nicole Babbs.
2. I have not been known by any other names.
3. My date of birth is 08/15/1986.
4. My Social Security Number is xxx-xx-xxxx.
5. My driver’s license number is 0190-08-1788.
6. My addresses from the date on which the first offense was committed to the date of this
petition are attached to this Petition as Exhibit A.
7. I am filing an expungement petition in Jackson County pursuant to Indiana Code § 35-38-
9-2. The details of all the convictions and criminal charges with no convictions in Indiana
that I am seeking to expunge are on the list of all my criminal records attached to this
Petition as Exhibit B.
8. At least five (5) years have passed since the date of my most recent Jackson County
conviction.
9. I have not been convicted of a crime within the previous five (5) years.
