3 A. I oversaw the whole operations of the facility.
4 So the day-to-day functions, you know,
5 communicated with nursi n g , h o u s e ke e p i n g , the
6 admi ss i ons department , communicated with
7 corporate, just making sure that the faci l i ty
8 ran appropri atel y .

16 A. We had multiple. So we had a nursing home. We
17 had as si st ed l i vi ng, and we h ad i nd epen den t
18 living. So the assisted living, there was an
19 as si sted l i vi ng di rect or, sh e managed th e

21 area, prett y much i nd epen dent l y. An d th en s he
22 woul d, you kn ow, d i s cu ss wi t h me i f s he h ad any
23 major con cerns and she that sh e wasn't abl e to

17 to t h e AL s i d e. And then yo u h a ve the di re ct or
18 of nursing, as well, who oversaw the quality of
19 ca re as pe ct.

21 with a lot of the customer service issues, any
22 co n ce rn s of, you kn o w, re s i de n t a bu s e J

1 So we worked very closely together. So it
2 was pretty much myself, as the administrator,
3 who was overall responsible, and the director
4 of nursi ng who, you know, assi sted me.
5 Q. So you said that in cases of resident abuse and
6 compl ai nts you and the di rector of nursi ng, at
7 l east for the nursi ng home si de, woul d be
8 responsi bl e for i nvesti gati ng that?
9 A. Correct, correct. And on the AL side, the
09:53 10 di rector woul d be the fi rst one that they
11 woul d -- she woul d know about i t and she woul d
12 communi cate wi th me. I woul d be responsi bl e to
13 assi st them and report as the admi ni strator who
14 oversaw pretty much the enti re campus i s how
15 the hi erarchy woul d be.
16 Bu t b e c a u s e m y o f f i c e wa s on the nursi ng
17 home end, her offi ce and her responsi bi l i ty was
18 on the AL si de. Does that make sense?
19 Q. It does .
09:53 20 A. Because i t was a real l y bi g campus. We have
21 the bi ggest campus out of al l the O'Nei l l
22 branches.
23 Q. So back up sl i ghtl y . Yo u sai d that you had
24 left O'Neill's in August of 2022. Why did your
25 empl oyment end?

1 A. I had a different opportunity at the VA. That
2 was kind of just dropped in my lap.
3 Q. So you left O'Neill's for the opportunity at
4 the VA?
5 A. Yes.
6 Q. And was that vol untary?
7 A. It was vol untary, yes. I put i n my noti ce. I
8 gave my 30- day noti ce. But they di d deci de to
9 l et me l eave earl y, but they pai d me out for my
09:55 10 30 days.
11 Q. You sai d they l et you l eave earl y Di d they
12 ask you, " Hey, do you want to l eave now as
13 opposed to 30 days?" Or di d they tel l you to
14 "S t a y h o m e J
bu t we wi l l sti l l pay you"
15 A. I am sorry J what was that?
16 Q. When you said you put in your 30-day notice,
17 but they let you leave your 30-day notice
18 earl y, di d they come to you and ask you, " Hey,
19 wo u l d you l ike to stop working the day-to-day

21 now that you have put i n your noti ce, we are
22 goi ng to have you stay home , b u t we wi l l sti l l
23 pay you"
24 A. They just told me to just, you know, go ahead
25 and go home and spend time with my family.

1 Q. Okay. Now, regarding your interactions with
2 any particular staff on the assisted living or
3 independent living side, like what would that
4 i nteracti on be l i ke on a day-to-day basis?
5 A. I try to make i t over to t h e AL s i d e probabl y
6 three to four times a week, you know, just to
7 round on them J see how things were going.
8 It is hard to ki nd of catch them because
9 there ' s four fl oors , and so they are ki nd of i n
09:57 10 and out of i t i s l i ke an apartment. Right?
11 So t h e y are i n and out of resi dent' s rooms. So
12 someti mes the best ti me woul d be ki nd of l i ke
13 meal ti me where they are i n the di ni ng room wi th
14 the resi dents.
15 Q. So i t was your practi ce to go over there around
16 meal ti mes to sort of catch more peopl e?
17 A. Ye s , I wo u l d try to catch them duri ng meal s
18 woul d probabl y be my best ti me to try to meet
19 wi t h them .

4 so, probably I would call it our average.
