On March 3, 2022, l accompanied John Van
Valkenburg to the hOSpice residence of Jeffrey Block located
at Suite Living Senior Care, 197 County Rd 32
W, Suite 9, Roseville, MN 55113. Also present in the room
were Alan Block and Jay Block, brothers of Jeffrey Block.
Jeffrey was alert and oriented. Although he
spoke softly, he responded to questions asked and offered comments.
Jeffrey is afflicted with
Parkinson's disease and cannot use his hands.
Upon inquiry he acknowledged that he understood the
Will and that he wished to secure its execution.
Two staff members, Chennea Thompson and Tiana
Chapman, arrived to act as witness. Upon the
affirmation request of Jeffrey Block, initialed each of the Will
J
page and signed his name on pages 6 and
7 of the Will.
