Order 2011-05-02-01, you may be eligible for foreclosure intervention, for the purpose of
resolving this foreclosure action. If you wish to be considered for foreclosure intervention, you
MUST, within thirty (30) days of service of this Notice upon you do the following:
1 Fully complete the form attached hereto as Exhibit “A.”
2 Return the completed form, TOGETHER WITH any information or data
pertaining to the loan secured by the mortgage upon your personal residence,
foreclosure of which is the subject of this action, or personal circumstance you
