Afterdue search, careful inquiry and diligent attempts at:Miami Dade Police Department, 9105 NW 25th St, Miami,
Miami-Dade County, FL 33172. I have been unable to effect service of process upon Miami-Dade Police
Departmentfor the folowing reasons:
5/30/2023 10:39 AM: I spoke with an individual who identified themselves as the person in charge and they
stated service not permitted. Officer stated if I'm try to serve police department all documents must Be taken
to downtown and if I trying to serve an officer I have to serve him directly. The officer stated I was looking for
works at 15665 Biscayne Blvd. Police officer did not let me through the doors and spoke to me through a
phone that they leave at main lobby.
Notary not required pursuant to F.S. 92.525(2).
Iam over 18 years of age, not a party to nor interested in this case and I have the proper authority in the jurisdictionwhere I effected
service, pursuant to Florida Statute Chapter 48. Per 28 U.S.C. § 1746, I declare under penalty of perjury under the laws of the United
States of America that the foregoing is true and correct.

During each pay period,a portion of the Defendant's salaryor wages as it becomes due shall be held
and not disposed of or transferred until further order of this court. The amount of salaryor wages to be
withheld for each pay period shall be made in accordance with the followingparagraph. This writ shall
continue until the Plaintiff's judgment is paid in full or until otherwise provided by court order.
Federal law (15 U.S.C. § 1671 - 1673) limits the amount to be withheld from salaryor wages to
no more than 25% of any individual defendant's disposableearnings(thepart of earningsremaining after
the deduction of any amounts requiredby law to be deducted) for any pay period or to no more than the
amount by which the individual's disposableearnings for the pay period exceed 30 times the federal
minimum hourly wage, whichever is less.

For administrative costs, the Garnishee may collect $5.00 against the salary or wages of the
Defendant for the first deduction and $2.00 for each deduction thereafter.
The total amount ofthe Final Judgment outstandingas set out in the Plaintiff's Motion is
$3992.74, pluspost-judgment costs, less any payments received.
FAILURE TO FILE AN ANSWER WITHIN THE TIME REQUIRED MAY RESULT IN
