PLEASE TAKE NOTICE THAT pursuant to the South Carolina Supreme Court Administrative Order 2011-05-02-01,
you have a right to be considered for Foreclosure Intervention.
To be considered for any available Foreclosure Intervention, you must communicate with an otherwise deal with

When submitting your request, or soon as possible thereafter, provide the following documentation:
1. A signed statement that details any hardship that may exist.
2. Evidence of income statement, disability income statements, leave and earnings statements;
3. An itemized list of all household expenses per month.
4. Verification of contact information that include home, cellular and employment telephone number, and e-mail
address.
5. Any other document(s) and/or information that will help determine your qualification for assistance through loss
mitigation options.
6. Amount you are able to pay toward arrearage.
